1.MOST EFFECTIVE最有效
Elimination - remove the hazard.消除危害
2.FAIRLY EFFECTIVE比较有效
Substitution, Isolation, Engineering Control替代、隔离、工程控制
3.LEAST EFFECTIVE 效果差
Administrative Control, i.e. 管理控制
reduce exposure time and limit, 减少暴露时间和极限
training, signage etc 培训签字

Recommendations for improvements建议
The focus of control column „What has to be done to avoid this?“ is the activity and the behavior of the operator
improvements that go beyond this sphere of influence in daily work need to be added to action item log / and corrective actions performed
Examples:
 - technical improvements on equipment
 - organisational optimisation

 
Completed JSA完成的JSA
Communicate the completed JSA to the associates 相关人员沟通

JSA Check
Regular check of critical steps of a JSA关键步骤定期检查
Scope: Continuous improvement in regard to safety安全持续改进

All steps are listed completely in the JSA所有步骤已经列出

·How long are you performing this activity?多久进行一次这项活动
·Were there any changes (equipment, material,..) ?有无变化(设备、材)
·Are there any problems with performing the activity?有没有问题
·Has there been a near miss to you or a colleague?你或同时以前是否发生过未遂事件
·Is the work safe from your point of view?你认为这项工作是否安全
·Are all required tools available (self made tools)?是否所有必要工具是齐备(自制工具)?
·Which defects, interruptions have to be expected?可能发生哪些过失或中断
·Do you have suggestions for improvement?提出过改进建议吗?
 
·An observation to control operator控制操作人员的观察
·Criticism of behavior of operator批评操作人员的行为
·Know-it-all / boss behavior无所不知老板行为

Finally
The most positive effect can only be achieved in cooperation with the operators 只有与操作人员协调一致才是最积极的效果
A moderator / discussion leader is required (supervisor level) 需要缓和人员/讨论的领导者(主管级别)
SHE specialist may review and optimize JSA but do not make the draft unrecognizeable
SHE专业人员可以评审JSA,使之优化,但不要作不认可的草稿
