GENERAL PROCEDURE

PURPOSE
目的

This procedure establishes the minimum requirements for the lock-out or tag-out of energy isolating devices.
It shall be used to ensure that the machine or equipment are isolated from all potentially hazardous energy and locked out or tagged out before employees perform any servicing or maintenance activities where the unexpected energization, start-up or release of stored energy could cause injury.

RESPONSIBILITY FOR IMPLEMENTATION OF PROCEDURE

It shall be the responsibility of the person who will perform work on a device or system to follow all established safety procedures including lock-out/tag-out of potentially hazardous energy.

         执行所有已规定的安全程序包括上锁挂牌是在设备上完成工作的人的责任。

It shall be the responsibility of management to insure that proper procedures are being adhered to.
Any questions arising from the need to apply these procedures shall be directed to management.

Any person deviating from or ignoring these procedures will be subject to discipline under plant rules.

NOTE:
Tag-out will not be acceptable where lock-out provisions are available.

LOCK-OUT / TAG-OUT PROCEDURE上锁挂牌的程序

All equipment shall be locked out or tagged out to protect against accidental or inadvertent operation when such operation could cause injury to personnel.
Do not attempt to operate any switch, valve, or other energy isolating device where it is locked or tagged out (Exceptions: see 1.3-(6) below).
Under no circumstances should an employee use a lock/tag other than their own.

              当有可能因为疏忽或意外操作引起事故的设备开启时，这样的所有设备必须上锁挂牌来避免这些事情发生。不要尝试去操作或打开任何开关、阀门和被孤立的有能量的被上锁挂牌的设备。（例外：1.3-(6)below）不管在什么情况下，只能自己挂自己的锁。

Make a survey to locate and identify all isolating devices to be certain which switch(s), valve(s), or other energy isolating devices apply to the equipment to be locked or tagged out.

查找和鉴别所有孤立的设备来确定哪个开关，阀门和孤立的能源设备需要进行上锁挂牌

CAUTION:
More than one energy source (electrical, mechanical, or others) may be involved!!!

(2)
Notify all affected employees that a lock-out or tag-out system is going to be utilized and the reason why.
The authorized employee shall know the type and magnitude of energy that the machine or equipment utilizes and shall understand the hazards thereof.

    通告所有相关的工人上锁挂牌要执行和原因。被授权的工人应该知道被利用设备能源的种类和大小，还应该知道他的危害。

(3)
If the machine or equipment is operating, shut it down by the normal stopping procedure (depress stop button, open toggle switch, etc.).

   如果设备正在运行，要通过正常的程序来关掉设备。

(2)        Operate the switch, valve or other energy isolating device(s) so that the equipment is isolated from its energy source(s).  Stored energy (such as that in springs, elevated machine members, rotating flywheels, hydraulic systems, and air, gas, steam, water pressure, etc.) must be released and/or restrained by methods such as repositioning, blocking, bleeding down, etc.
通过控制开关，阀门和有能量的设备来断开能力源。有能量的零件（例如弹簧，转动的齿轮，液压系统，空气，天然气等等）必须被释放或者通过一定的方法来束缚。
(3)        Lock-out and/or tag-out the energy isolating devices with assigned individual lock(s) or tag(s).  When a tag-out device is used, it shall be affixed in such a manner as will clearly indicate that the operation or movement of energy isolating devices from the “safe” or “off” position is prohibited.
上锁或者挂牌必须针对每一个有能量源的设备。当有一个挂牌的设备在使用的时候，这个牌子必须是一个很现眼的标志，来告诉别人关闭或停掉这个正在运转的设备是禁止的。
(4)        After assuring that no personnel are exposed, and as a check on having disconnected the energy sources, operate the push-button or other normal operating controls to make certain the equipment will not operate.  If there is a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation shall be continued until the servicing or maintenance is completed, or until the possibility of such accumulation no longer exists.
                     
CAUTION:  Return operating control(s) to “neutral” or “off” position after the test.
警告：在测试后一定要把操作状态回到中间或者关闭状态
(5)        The equipment is now locked out or tagged out.
设备现在已经挂牌上锁了。

1.4        RESTORING MACHINES OR EQUIPMENT TO NORMAL PRODUCTION OPERATIONS
               恢复设备或者机器到正常的操作状态

(1)        After the servicing and/or maintenance is complete and equipment is ready for normal production operations, check the area around the machines or equipment to insure that no one is exposed.
    在维修完毕以后开启设备以前，必须检查一下以确认是否有人员处在危险的位置。

(2)        After all tools have been removed from the machine or equipment, guards have been reinstalled and employees are in the clear, remove all lock-out or tag-out devices.  Operate the energy isolating devices to restore energy to the machine or equipment.
                  在维修工具取走以后，防护装置要重新放回原来的位置，清点人数，然后把锁或警示牌去掉。
IMPORTANT:  When the authorized employee who applied the lock-out or tag-out device is not available to remove it, the device may be removed, provided the following procedure is adhered to: (1) the employer verifies that the authorized employee who applied the device is not at the facility; (2) the employer makes all reasonable efforts to contact the authorized employee to inform him/her that his/her lock-out or tag-out device has been removed; and (3) the employer ensures that the authorized employee has this knowledge before he/she resumes work at that facility.
重点; 当被授权进行上锁挂牌的人员不方便或者不可能去掉警告牌或锁时，必须按照一下的规定：（1）确认去执行工作的人是否对这个工作熟练。（2）被授权的人员必须尽最大可能去确认警告牌或者锁已经被拿掉。（3）别人去拿掉的时候，被授权的工作人员必须确定这个人是否有这方面的知识和经验。

1.5        PROCEDURE INVOLVING MORE THAN ONE PERSON程序包含的人员

        If more than one individual is required to work on a particular piece of equipment, each shall place his/her own personal lock-out and/or tag-out device on the energy isolating device(s).  When an energy-isolating device cannot accept multiple locks or tags, a multiple lock-out or tag-out hasp must be used.  As each person no longer needs to maintain his or her lock-out protection, that person will remove his/her lock from the energy-isolating device.
如果超过一个人以上的要对设备的一部分进行操作或维修，每一个人操作的地方都要求上锁挂牌。当有些设备即要上锁又要挂牌时，必须又这样的工具。当所有的人都维修或操作完毕以后才能取锁通电源。

1.6        SHIFT OR PERSONNEL CHANGE换班或者变动

        If the lock-out/tag-out situation should extend beyond the current work shift and work is expected to be completed before the following day, the in-coming employee(s) assigned to the job must apply his/her lock/tag prior to removal of the off-going employee(s) lock/tag.
               如果已经上锁挂牌的工作需要到下一个班或者第二天才能完成，来接替这个员工来工作的人必须先上锁，然后被接替的人才能取下锁。
        In any case, in the event of lock/tag change, it must be assured that the device is at zero energy state before work resumes.
              在任何情况下，如果发生上的锁或者挂的牌需要摘掉，在重新开始上锁挂牌前必须保证能量源是在0状态。

1.7        OUTSIDE PERSONNEL外部人员

        Outside personnel, such as contractors, must be made aware of the lock-out/tag-out procedures of the Company.  This is best accomplished during the initial meeting with outside personnel where a copy of the Company’s lock-out/tag-out program can be presented and discussed along with other safety program requirements.
                对于外部人员，例如外包商，必须清楚本公司的上锁挂牌的程序。 对外包商的上锁挂牌的培训必须在刚进入公司的培训上和其他的安全规定一起给外包商进行培训。


1.8        HARDWARE REQUIREMENTS锁具的要求

(1)        All lock-out/tag-out devices shall be supplied by Corhart.
所有上锁挂牌的锁具标识必须由公司提供。

(2)        All lock-out devices and tag-out tags for Corhart shall be of the same configuration and color.  This will help in minimizing the possibility of the meaning and purpose of the device being confused with some function other than a lock-out situation.
公司内部上锁挂牌的工具必须是统一的规格和颜色。这样会帮助我们尽可能的减小上锁挂牌的工具和其他的功能工具混淆的可能行。
(3)        All  lock-out/tag-out devices must be used exclusively for lock-out applications and no other.
上锁挂牌的工具必须是专用的，不能用作其他工作。

(4)        The signal word DANGER shall be readable at a distance of five feet on tag-out tags.
标有“危险”的标识牌必须在5英尺以外就可以清楚的看见。
(5)        Tags must be durable and remain legible in use.
标识牌必须是耐用的、在使用过程中可可以方便看到的。
(6)        Lock-out locks shall be purchased, keyed separately.  A spare key will be held by the Maintenance Dept.  in the safety lock-box in the Maintenance Advisor’s office and will be the only means used to remove a lock-out inadvertently left applied.  NO LOCK ON A LOCK-OUT DEVICE IS TO BE CUT-OFF.  Additional checks are to be made by the Maintenance Advisor or project manager prior to removing an abandoned lock-out and restarting equipment.  (See Section on Restoring Machines or Equipment to Normal Production Operations).
锁具必须是采购的而且钥匙必须是单独的。必须要有备用钥匙，而且备用钥匙必须在保存在维修主管的锁具盒里，备用钥匙必须是在没有其他办法解锁的时候才能使用。
(7)        A listing of lock numbers, key numbers, and their assigned users shall be maintained in the Maintenance Advisor’s office.
锁具数量、钥匙数量和授权使用人的清单必须张贴在维修主管的办公室内。
(8)        Each lock-out lock shall bear the name of the employee to whom it has been assigned.
每一个上锁的锁必须有上锁员工的姓名。

1.9        EMPLOYEE TRAINING员工培训

        All affected authorized employees will participate in the lock-out/tag-out training program.  The training program shall be formulated utilizing the following, but not limited to:
             所有授影响的员工必须参与上锁挂牌的培训。培训的内容必须包括但不限于下列内容：
(1)        Instructional Outline:  The outline of instructions will ensure the uniform subject coverage for all employees and will allow review by outside agencies of the scope of the training.

(2)        Acknowledgment Statement:  An acknowledgment statement, indicating that the information covered in the training class was understood, shall be signed by all employees attending lock-out/tag-out training.  Any employees indicating they did not grasp the information covered in the training class shall be immediately retrained.
确认表： 所有参加培训的员工一定要亲自签字。有员工对培训的内容有不明白的一定要重新培训。
(3)        Required Subject Matter:  The following areas are required to be covered in the lock-out/tag-out training program:

a)        Purpose for the utilization of the lock-out/tag-out procedure.
上锁挂牌程序的目的
b)        Explanation of extension of lock-out/tag-out procedure to include energy sources lock-out other than just electrical disconnect, i.e., unanticipated movement of component parts, e.g., springs under tension, residual pressure in lines and uncompleted cycles or strokes of machinery.
对于上锁挂牌程序的解释，包括能量源的种类，不可预料的运动部件，弹簧，残余在管道内的压力，换班等等。

c)        Proper application and utilization of lock-out/tag-out devices.

1.10        PERIODIC INSPECTION定期检查

        A periodic inspection of the energy control procedures shall be conducted at least annually to ensure that the procedure and the requirements of this standard are being followed.
               能量源定期检查至少要一年一次。

1.11        RETRAINING多次培训

        Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures.
             只要设备、人员或者工艺发生变化必需对所有人员进行再次培训。
        All employee training that has been accomplished shall be certified and kept up to date.  The certification shall contain each employee’s name and dates of training.
              所有员工培训完后必需再次确定，包括培训员工的姓名和培训日期。
